In 2003, Professor Bengt Fellström wasn't trying to start a longevity company. He was studying kidney disease at Uppsala University Hospital — one of Sweden's oldest medical institutions, founded in 1708. Like most kidney specialists, his work focused on a narrow question: why do some patients' kidneys decline rapidly while others remain stable for decades?

The answer, he kept noticing, had less to do with the kidneys themselves than with the small blood vessels feeding them. And the most important molecule controlling those vessels — a colorless gas called nitric oxide — was something most cardiologists, even those who knew about the 1998 Nobel Prize, weren't paying enough attention to.

"It was hiding in plain sight," Fellström later said in a Swedish science publication. "The chemistry was known. The mechanism was known. But the question of how much we make as we age — and what to do about it — that was almost untouched."

The molecule the world almost forgot

Most people remember the names Furchgott, Ignarro, and Murad — if they remember them at all — from a single sentence in a high school biology textbook. The three American scientists won the Nobel Prize in Physiology or Medicine in 1998 for discovering that nitric oxide acts as a signaling molecule in the human cardiovascular system.

The discovery was revolutionary. Nitric oxide tells the smooth muscle in artery walls to relax, opening them up to allow more blood flow. It's why nitroglycerin — used to treat chest pain for over a century — works. Without anyone knowing why, doctors had been giving patients a drug that converts to nitric oxide in the body.

But after the Nobel buzz faded, the molecule slipped back into the background of medical research. Most physicians filed it under "interesting biochemistry" and moved on.

That's where Sweden enters the picture.

The Karolinska connection

While Fellström was working in Uppsala, a parallel line of research had been building 70 kilometers south, at the Karolinska Institute in Stockholm — the same institution that awards the Nobel Prize in Medicine each year.

For more than two decades, a group of Karolinska researchers had been quietly mapping something the original Nobel laureates hadn't fully explored: a second, separate pathway by which the body produces nitric oxide — one that doesn't depend on the cardiovascular system at all.

It starts in your mouth. Specifically, in the bacteria living on the back of your tongue.

When you eat foods rich in dietary nitrates — leafy greens, beetroot, certain root vegetables — these bacteria convert nitrates into nitrites. The nitrites travel through your bloodstream and, under the right conditions, become nitric oxide. It's a parallel pathway, completely separate from the one Furchgott and his colleagues had described.

And it appears to weaken with age.

What changes after 50

By the time most people reach their fifties, the natural production of nitric oxide via the L-arginine pathway has declined significantly — some estimates suggest by as much as half compared to a person in their twenties. The dietary pathway, the one Karolinska researchers were studying, becomes more important precisely when most people are eating fewer of the foods that feed it.

"You can think of it as an aging engine," Fellström explained in a Swedish documentary. "When we're young, the engine has two fuel lines. As we age, one starts to clog. If we don't feed the second line — the dietary nitrate line — we end up running on a fraction of the fuel we used to have."

This was the realization that connected Fellström's work in Uppsala with the Karolinska group in Stockholm. And it became the foundation for what would become a peer-reviewed paper in Nutrients, a major scientific journal, in late 2024.

What this doesn't claim
